Pain Management North, LLC needs a new banner ad

Pain Management North, LLC

Pain Management North, LLC is a pain management clinic based in Atlanta that helps residents of all 159 counties in Georgia get better control of their back, neck and sciatica pain.

I am looking for a set of 3 banner ads - 728x90, 300x250 and 160x600. File size should be 25k or less. I am looking for the use of animated gif/jpeg or flash. I would like a button included in the banner with a call to action. The keywords I like best are pain management clinic, pain clinic, pain management, sciatica, back pain and neck pain. Please feel free to refer to http://www.painmanagementnorth.com for any copy or graphic ideas, but don't feel limited to what you find there. I have included a few images below, including our logo, that have been well-received previously. Feel free to use them or not. I would love to have a good photo or illustration for sciatica, but have been unable to find one that I like. Please include the url somewhere in the banner.

In general we are trying to appeal to a demographic that is older than 30, well-educated, slightly more female than male, located anywhere in Georgia.
