In our program, we have a decision making model that incorporates multiple stakeholders when making decisions about patient care. One is the evidence, one is the patient, and one is what the clinician brings. Our current graphic works well, but our "patient" focuses just on the patient and family, and the family is stereotypical of the traditional family. Physical Therapist also have an impact on the community and may create programs that have effects that extend beyond the patient/Family. We would like a revision of our current graphic to include the broader community.
I have included the original model and a sketch of the new model, we see the patient and family still in the center, and prominent, but we also want more people to indicate the greater community,and with less emphasis. If the family is in black ( some other way to make them prominent) in the center, then we see the community members in different shades of gray getting smaller and fading away the further away we get from the center.
We are not set on the look of the people, or colours , but we want to keep the circular graphic. We also want to change the order and text as outlined in my very rough sketch. The text in the sketch is the text we would like in the new model - Theory, Assessment & Intervention, Research & Evaluation, and Outcome. We need to keep to a similar model as shown in the original and sketch, but there is definitely leeway for creativity. We probably need more people than is depicted in my sketch, or not? We want to focus on the patient, but make sure we include the community.
