I want to create a 90 Day journal. Each day needs to include two pages. One page will be titled THOUGHT DOWNLOAD and have lines the facing page will have two "models" where they can fill in their CIRCUMSTANCES, THOUGHTS, FEELINGS, ACTIONS RESULTS C: _____________________________ T: _____________________________ F: _____________________________ A: _____________________________ R: _____________________________

On the inside cover I want the full model with words (see attached book for full model jpg-but please design as wanted) I want the cover to be black and say 90 Day journal (Glossy and thick) Inside is very simple thick paper Bound with spiral but high end binding 5x7 is the size I've included the interior of my Self Coaching book so you can see the model vs. blank model
